For Service Holder:
•    Original passport travel time to valid last seven months
•    02 copies recent original photos from studio lab print (back ground white) size 3.5cm x 4.5cm, (frontal
 view, without dark glasses and head gear (except for religious purpose) scan photo scan paper not allowed.
•    02 copies visiting card
•    Bank solvency certificate
•    Original last seven months bank statement (Individual 85,000 BDT & Family 1,30,000 BDT)
•    Company leaves letter

If there is no (Husband & wife) name mentioned in passport in that time original marriage certificate or Nikah Nama needed.

Medical Visa:
•    Copy of recent certificate of referral or a letter (preferably original) from a local doctor / hospital (in official pad with contact number, sign & date), where the patient is being treated which states the patient’s condition; (for first-time traveler)
•    In case if the patient receives treatment in a foreign hospital, proof of previous medical treatment in foreign hospital is required; (for first-time traveler)
•    Recent case summary of medical reports of the patient issued by the hospital; (for first-time traveler)
•    Copy of an appointment letter from a hospital in Thailand mentioning appointment / admission date & accompanying person name(s)
